Leave of Absence Form for Holidays
Where possible, please try to make medical appointments after school or during the holidays. However, we recognize this is not always possible.
If your child has a doctor, dentist, hospital or educational appointment, you can now let us know by emailing the office team at office@burpham.surrey.sch.uk, giving us all the necessary details including what the appointment is for. You do not need to give us a copy of the appointment letter.
